Instructions for Lucas/Little River Parents and Students trying out for Spring Athletics – 2016-2017
ATTENTION: 6th grade students are now eligible to tryout for all sports starting this Sping!
Your child’s Physical Form and Concussion Form must be turned in to the Athletics Office or ISS Room by Monday, February 13th, 2017. There will be a bin for collection of these items. You might want to staple these items together. Copies of these forms are on the Lucas Middle School website — lucas.dpsnc.net and click on the Athletics tab and then the Forms tab. If you have further concerns or questions, please contact your coach or the Athletic Director (AD), Rock Magis at [email protected].
2017 Dates for Spring Athletic Team Try-outs:
Thursday, February 16th – Time 2:45 p.m.
Please make a copy of your child’s physical and concussion form!
Keep this on file at home!
Your child's forms will be processed and other eligibility items (grades and attendance) will be checked after the physical and concussion form have been turned in together. On the first day of tryouts, if your child is eligible, the coach will have your child’s name on a list to show that he/she is able to go through the tryout process. If your child is ineligible, we will notify you of the situation and allow your child to know what prevented him/her from trying out for Spring Athletics 2016-2017. Please make sure that your child’s physical and concussion form are turned in to your coaches or AD on time, so that we can finish out the process of eligibility. If these forms aren’t turned in on time, there is no guarantee that your child’s paperwork will be processed by the first day of tryouts.
IMPORTANT REMINDERS:
Make sure your Child’s Name, DOB and any other important information are complete at the top of physical. Make sure that the physician has marked a box at the CLEARANCE section. Confirm that there is a signature of physician, date of exam and office stamp on the physical. Be sure that parent and athlete sign, date and add a current phone number.
